Job Description:
The Advanced Practice Nurse (APN) provides on-going evaluation and treatment to Mental Health Partners (MHP) clients with medications in addition to other forms of therapeutic interventions. All services provided will remain true to the MHP Guiding Principles and PSR Service Delivery Guidelines. In collaboration with staff psychiatrists, the APN provides psychiatric care to designated clients, including patient assessment and diagnosis, medication management, evaluation and consulting and collaborating with and referrals to outside providers
Essential Duties:
- Performs assessment of clients health and medical condition
- Reviews patient history and obtains further information as necessary
- Develops a diagnosis and treatment plan for clients and implements the treatment plan
- Initiates psychotropic interventions and evaluates effects of medication interventions and medications as indicated
- Orders and monitors appropriate laboratory tests, blood work and/or other tests as clinically indicated
- Collaborates and consults with other internal staff and outside providers (DSS, Dept of Corrections, primary care providers, health dept., homeless shelter, etc.) regarding present and past treatment of clients and treatment planning
- Adjusts medications and writes/renews prescriptions
- Acts as liaison with clients Primary Care Physician (PCP).
- Facilitates medical care and when appropriate, facilitates transfer of care to the PCP
- Participates as an active member in multi-disciplinary teams providing comprehensive case management services to clients
- Refers clients with medical complications to outside providers as necessary
- Visits non-mobile clients in nursing homes, assisted living, or in clients homes
- Responds effectively to client needs and problems
- Performs day call duties
- Initiates and maintains positive interactions, provides timely response to phone calls, e-mails and other requests and regularly participates in team meetings
- Acts as a resource to EPS for problem solving and providing prescriptions.
- Completes documentation of services and treatment plans consistent with clinical and administrative policies and procedures
- Promotes and demonstrates MHPs mission, vision and values through both behavior and job performance on a day to day basis
- Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.
- Regular and consistent attendance is required to perform other essential functions of the job
- Other duties as assigned
Specialized Duties:
- May conduct wellness related classes for clients and/or staff
- May be involved in teaching psychology and social work interns, medical students, PA students, and nurse practitioner students
- May be involved in outreach and education with partnering agencies
Minimal Qualifications:
- Current RN licensed to practice in Colorado and 6 years relevant work experience
- MSN, Psychiatric Mental Health Nurse Practitioner degree required
- Prescriptive Authority in the State of Colorado
- Current CPR card
- At least 5 years relevant experience (working in an inpatient or outpatient setting with clients who have serious psychiatric difficulties) and specialized status in one of the following disciplines: Advanced Practice Nurse or Nurse Practitioner preferred
Knowledge, Skills and Abilities:
- Excellent customer service and interpersonal skills
- Excellent knowledge and skills in current psych-pharmacology, psychiatric diagnosis, recovery principles, and psychiatric and evidence based interventions
- Ability to work independently as well as in a team setting and with a larger treatment team
- Must be competent in working with people of diverse cultural and ethnic backgrounds
- Highly professional and able to manage difficult situations
- Ability to organize and prioritize workload
- Demonstrated computer proficiency including use of electronic health record and Microsoft Office products and ability to learn additional tools
- Ability to travel to various MHP sites and/or community locations
- Bicultural/Bilingual (Spanish) preferred
