Nurse Practitioner

Responsibilities:

Applies advanced knowledge/clinical skills necessary to assess/manage the healthcare of chronically ill geriatric members in the Senior LIFE program. Focuses on empowering members with the goal of maximizing quality of life. Assess, initiate and coordinate member care on a daily basis under the supervision of the attending physician Act as a liaison with physicians, nurses  and other healthcare professionals, providing education, communication and support for healthcare staff, members and families

Qualifications:

Must be a PA licensed CRNP from an accredited university A minimum of one year experience  with experience in geriatric care preferred
