Job Description

Health at Home is now hiring a Nurse Practitioner for a high end CCRC in Scottsdale, AZ.

The Nurse Practitioner is responsible for providing primary or palliative care and maintaining continuity of care. Establishes diagnosis and treatment of common acute and common chronic illness with health promotion and disease prevention.

Essential Job Functions/Responsibilities:

A. Provides general medical care and treatment to patients under direction of physician. Performs physical examinations and preventive or palliative health measures within prescribed guidelines and instructions of physician.
B. Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
C. Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
D. Prescribes pharmacologic, non-pharmacologic and other forms of treatment or related therapeutic procedures.
E. Outlines and maintains a process for ensuring the receipt, documentation, and conveying of results to the patients, caregivers and clinical or hospice staff for all ordered tests and referrals.
F. Performs certain laboratory studies including the drawing of blood samples, performing urinalysis, stool examination, EKG, bacteriologic and cytological smears and cultures, skin tests, and blood glucose determinations.
G. Performs therapeutic procedures including injections, immunizations, cleansing, suturing, dressing wounds and decubiti, inserting nasogastric tubes and catheters, irrigation of ear canals, and administration of oxygen.
H. Diagnoses and treats acute medical problems and manages chronic or palliative conditions; makes necessary rounds.
I. Performs pre-admission physicals on potential patients and makes recommendations for level of care OR Face to Face assessments of referred patients for admission to the Hospice Benefit and makes recommendations for level of care implementation.
J. Issues orders for medication, treatment, and laboratory tests under the guidelines and mandates of the Board of Medical Examiners and under the direction of a physician.
K. Consults with members of the interdisciplinary team to integrate the patient’s plan of care.
L. Provides patient and caregiver education and counseling.
M. Maintain open communication with medical director and other physicians.
N. May require visits within the patient’s home environment to meet patient needs.

Position Qualifications:

1. Master’s Degree in Nursing
2. Graduate of an accredited NP program required.
3. Requires a valid, unrestricted state NP certification and RN license.
4. Minimum 1-2 years’ experience as an NP.
5. Previous with geriatric patients is preferred.
6. Understanding of Medicare and health plan benefit plan structures a plus.
7. Candidate should have excellent people skills, with superior written and oral communication skills, and the ability to work both as a team member and independently.
8. Intermediate computer literacy, including medical record documentation.
9. Demonstrates excellent observation, assessment, nursing judgment and communication skills.
10. Ability to provide accurate written documentation in a timely manner.
11. Self-directed with the ability to work with little supervision.

Certifications & Licenses:

• Successful completion of a Nurse Practitioner Program from an approved School of Nursing.
• Successful completion of certification exam.
• Current license to practice in the state as a Registered Nurse.
• Verification of approval from the State Board of Medicine and the State Board of Nursing to practice in the state as a Nurse Practitioner
