Physician Assistant/Nurse Practitioner – Full Time
Denver Metro Area

JOB SUMMARY:

We are a Primary Care Medical Practice where patient quality of life is our primary goal. We are currently looking for a Physician Assistant or Nurse Practitioner to be a part of our team, serving patients in clinic and select skilled nursing /rehab facilities in the Denver Metro Area.

Our Physician Assistants and Nurse Practitioners assess and treat acute/chronic medical problems. Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures. They coordinate patient care as well as write prescriptions.

We have been recognized by NCQA as a Level III Patient-Centered Medical Home practice and are dedicated to providing our patients continuum of care with the use of a fully integrated Electronic Health Record available real time in all of our places of service.

This is a team oriented environment. The ability to build a rapport and cooperative relationship with patients and nursing home facilities is essential.

REQUIREMENTS:
• Completion of an accredited program for Physician Assistants/Nurse Practitioners and currently licensed in the State of Colorado.
• 1 plus years in practice is preferred.
• Prescription licensure required.
• Training/experience in a primary care/family practice setting desirable, experience in skilled nursing facilities is a plus.
