Founded as a small family practice in Escondido 1932 by Dr. Martin B. Graybill, today we’re the region’s largest Independent Multi-specialty Medical Group. Our capacity to provide for our patients through the full range of care continues as we look to fill the following position:

Chronic Care PA or NP with CDE (Certified Diabetes Educator) teaching experience in a high-volume family practice, internal medicine environment. Bilingual in English/Spanish required. Ideal candidate will have excellent communication and clinical skills with proven competency in the treatment and education of patients with predominantly Type 2 diabetes. Individual and small group instruction and management will be required. Disease prevention in a collaborative environment which focuses on individual patient needs emphasized. High degree of autonomy. The ideal candidate will have a family medicine or internal medicine patient care background with disease management emphasis.

Skills/ Requirements
The incumbent must hold a current California (PA or NP) license and be ACLS and CPR certified. Bilingual in English/Spanish required. CDE (Certified Diabetes Educator) teaching experience.
