Job Description

Serves as an advanced practice nurse providing primary level of care in collaboration with physicians and other members of the health care team to a specific patient population in the outpatient and/or inpatient setting. Candidate will function in ambulatory and inpatient settings in a downtown San Antonio clinic location.

Candidate will be responsible for evaluation and management of patients with heart failure and cardiovascular disease primarily in an ambulatory setting. Some coordination and transitional care for inpatients may be required. The successful candidate will demonstrate critical thinking skills in clinical decision making by the identification, evaluation, and management of the individual needs of patients/families for those with heart failure. Expertise in caring for patients across the age continuum will be important.

The successful candidate will have experience and expertise in managing acute and chronic heart failure in accordance with current guidelines and established collaborative practice protocols with an emphasis on health promotion, disease prevention and avoidance of readmissions. The candidate will be required to participate in the Identification of unstable or complex patient care situations and will facilitate appropriate evaluation and intervention with the Cardiologist in the clinic or hospital setting.
This position may require extended hours or include hours on call.

State licensure as an Registered Nurse and graduation from a Nurse Practitioner program or Master’s degree in Nursing with certification.

Ideal candidate will be Master’s prepared with current TX licensure, board certification and experience in Cardiology and participation as a lead or team member in treating heart failure patients. Candidate should have EMR experience with Allscripts EMR being optimal. Maintains an advanced knowledge base and skills in health promotion and maintenance, disease prevention, physical and psychosocial assessment, and management of health and illness in the health care of individuals. Potential candidate must have excellent verbal and written communication skills.
