Full-Time Nurse Practitioner – Cerritos, California

Job Description

Los Angeles California based Independent Physician Association (IPA) is seeking a Nurse Practitioner to complete health risk assessments on plan members in the home, hospital, SNF or clinic setting.  We have an immediate opportunity for a super-star Nurse Practitioner demonstrating high initiative, and able to relate well with patients, especially geriatric, effectively identify and review chronic conditions, and coordinate with PCP to ensure comprehensive patient care at our downtown LA location.

 

Primary Responsibilities:

  • Perform physical examinations and preventive health measures in individual homes, hospitals, SNFs or clinic settings
  • Complete thorough health assessments forms
  • Ensure assessments comply with Medicare documentation guidelines
  • Conduct, order, interpret, and evaluate diagnostic tests
  • Record physical findings and formulate plan and prognosis
  • Support Risk Adjustment Clinical strategy
  • Implement and drive CMS 5 Star initiatives
  • Maintain effective and friendly communication with supervisor, physicians, and patients
  • Strong clinical assessment and documentation skills
  • Proactive and able to work independently with strong initiative and organizational skills
  • The ideal candidate will be savvy and direct with a warm and compassionate demeanor

 

Requirements:

  • Current California Nurse Practitioner license
  • Current California driver’s license
  • 2 to 4 years direct patient care experience, specializing or skilled in Gerontology preferred
  • Managed care experience a plus
  • Thorough understanding of Medicare HCC guidelines
  • Working knowledge of current ICD-9-CM and CPT coding (ICD-10 a plus)
  • Familiarity with CMS 5 Star measures
  • Bilingual preferred: SPANISH or KOREAN strongly desired
