Landmark Connect extends the mission of Landmark Health by transforming care delivery for patients that reside in skilled nursing facilities (SNFs) and long-term care facilities. Landmark Connect embeds high-quality primary care providers (MD and/or APPs) in facilities as dedicated support to treat residents in-place. By placing primary care resources in facilities, we aim to deliver better health outcomes for patients, improve facility performance on quality metrics, and improve patient and family satisfaction.

We are looking for providers who are passionate about improving the quality of care for patients residing within facilities and helping facilities improve and maintain high performance on quality metrics and standards.

Responsibilities
As a Landmark Connect provider you will manage a panel of patients within a designated facility. You will work closely with the nursing staff at the facility, the Medical Director, and with other members of the patient’s care team in managing patient care. Care will include performing admission H&Ps, conducting regular interval and medically necessary visits, managing the patient’s plan of care, follow-up with patient’s family, and day-to-day communications and follow-up with facility staff. You may also be involved in the development and/or implementation of medical management policy, clinical protocols, and quality management programs.

As a provider, you will be expected to perform the following activities:
• Establish and maintain effective relationships with patients, their family and legal representatives, facility staff, and other collaborating physicians/providers.
• Diagnose, prescribe, and treat within the scope of the Nurse Practitioner and consistent with standard of care.
• Collaborate with facility staff to enable medical necessity determinations for requested medical services.
• Following the visit, promptly document and communicate any pertinent information to the facility staff
• Understand and comply with long-term care policies and procedures, including those that regulate nursing facilities and assisted living communities.
• Participate in telephonic on call rotation

Qualifications
• Completion of Nurse Practitioner (NP) / Physician Assistant (PA) accredited program.
• Minimum of two (2) years clinical experience, desired.
• Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.
• Strong interpersonal skills
• Passion for geriatric care and patients
• Knowledge of clinical standards of care
• Knowledge of CPT and HCPC codes is helpful
• Awareness of Medicaid/Medicare contracts and benefit systems is helpful
