Job Description

Daily Duties:

*Perform routine family care medicine

*Promote preventative care and wellness

*Collaborate with supervising physician

*Develop and administer care plans for patients with chronic conditions

*Supervise support staff

*Perform minor procedures

Qualifications:

Utah License, Board certification, DEA license, Utah controlled substance license, Buprenorphine management experience helpful
