Job Description
I. TITLE: Certified Registered Nurse Practitioner (CRNP)
II. PRIMARY FUNCTION: In the VNA Family Health Center, the Certified Registered Nurse Practitioner (CRNP) provides primary healthcare and illness treatment as well as related health education and guidance, referrals, and appropriate follow-up.
III. QUALIFICATIONS:
Education: Registered Nurse who has completed the Nurse Practitioner Program. Masters Degree in Nursing preferred. Advanced Practice Nurse Certification is required. Prescriptive privileges are preferred.

A. Ascending: Professional Nursing Services; Practice & Operations Manager
B. Descending: Not applicable
C. Coordinating: Registered Nurses, Volunteers, Receptionist, Medical Assistant, RN Case Manager
V. RESPONSIBILITIES AND ESSENTIAL DUTIES:
- Maintains confidentiality in dealing with VNA-Community Services, Inc., its customers, vendors, clients, and patients. Complies with HIPAA regulations and agency policies related to HIPAA compliance.
- Utilizes the nursing process and advanced practice education and certification to perform pediatric evaluation and care – physical examination, developmental assessment, screening tests, and psychosocial assessment.
- Using evidence-based guidelines diagnoses and treats common childhood illnesses. Follows the guidelines and procedures for handling common medical problems and consults with or refers directly to the appropriate physician when a problem arises which is not covered by approved guidelines.
- Is an integral member of the team-based care model and patient-centered medical home. Assists the healthcare team in formulating the plan of care and setting priorities based on the needs of the patient and family.
- If assigned, shares responsibility for telephone on-call after clinic hours and weekends, answers questions or concerns of families and sends child/family to the emergency room if necessary. Handles all calls in a timely manner following the Health Center policy.
- Maintains clinical skills and acts as a clinical resource person for families and clinical role model. Effectively collaborates with all members of the health care team.
- Identifies community health, social, and economic resources for families and connects patients to available community resources.
- Provides patient and family education. Provides health education and promotion as well as disease prevention education in a culturally and linguistically appropriate manner. Has familiarity with general problems of vulnerable populations.
- Advocates for healthy lifestyles and is committed to assisting patients with maintaining or improving overall health. Develop individualized care plans including self-management and outcomes.
- If assigned, works as a clinical preceptor for nursing students who rotate through the Health Centers.
- Complies with program objectives and operational expectations.
- Assures OSHA safety standards are met (Personal Protective Equipment (PPE) required as appropriate.)
- Assists with responsibility and compliance for clinical and statistical records and data collection associated with the program and clinics.
- Monitors and participates in quality assurance activities. Attends monthly doctor meetings, and regular team meetings and participates in daily team huddles.
- Communicate with clinic staff about patient-specific items via messages in the Electronic Health Record (EHR.) Communicate via email any non-specific patient-related concerns or suggestions to improve the overall quality of patient care.
- Contributes to the ongoing responsiveness and appropriateness of practice standards, protocols, and quality care.
- Accepts other assignments as requested or directed by VNA for work within the scope of the employee’s general job description, education, and/or licensure.
