Job Description
Salary Range: $135,000 – $153,231 a year
Position Summary:
We are seeking a dedicated Nurse Practitioner to join our outpatient nephrology team. This role focuses on managing patients with chronic kidney disease (CKD), end-stage kidney disease (ESKD), and related conditions in both the clinic and dialysis settings. The NP will work closely with nephrologists and interdisciplinary care teams to ensure comprehensive, high-quality care.
Key Responsibilities:
Outpatient Clinic Duties:
- Conduct new and follow-up visits for patients with CKD, uncontrolled hypertension, electrolyte imbalances, and related conditions.
- Perform patient assessments, order and interpret labs, adjust medications, and provide education and care coordination.
- Document clinical encounters promptly in the EMR.
- Participate in patient counseling related to disease progression, lifestyle modifications, dialysis modality options, and transplant referral.
Dialysis Unit Duties:
- Round regularly at affiliated dialysis centers (hemodialysis).
- Monitor and manage dialysis patients in collaboration with the nephrologist and dialysis staff.
- Review labs, assess treatment adequacy, manage anemia and bone-mineral metabolism, and respond to patient issues.
- Serve as a resource to dialysis nurses and staff on nephrology care questions and patient needs.
Qualifications:
- Master’s or Doctorate degree in Nursing from an accredited NP program.
- Current Nurse Practitioner licensure in the state of Oregon.
- National certification as an Adult-Gerontology or Family Nurse Practitioner.
- Current BLS certification; ACLS preferred.
- Minimum 1 year of experience in internal medicine, nephrology, or chronic disease management preferred. New graduates with relevant clinical training and strong interest in nephrology will be considered.

- Knowledge of ESRD, CKD, and dialysis care protocols (training will be provided if needed).
Work Schedule:
- Monday through Friday, 4 ½ days/week, daytime hours.
- No hospital coverage or overnight call.
- Travel between clinic and dialysis units (within a defined local area) may be required.
