House Calls Nurse Practitioner
Riverside Health System is recruiting a House Calls Nurse Practitioner to join the interdisciplinary consultation team in Newport News, Virginia. You would work in a collaborative approach with physicians to address complex pain and symptom management, goal-setting, communication and care transitions. Ideal candidates maintain current knowledge of primary care and evidence-based medicine practices and demonstrate skills in teamwork, collaboration and communication with both staff and patients/families.
Riverside House Calls Practice provides primary healthcare services to home-limited adults who cannot otherwise access regular medical care. We bring physicians and nurse practitioners trained in primary care and focused on the care of older adults to the patients’ homes so they can receive care in the place they are most comfortable. Education: Credentialed Nurse Practitioner in good standing.
Licensure/Certification: Virginia Nurse Practitioner and licensed RN in good standing.
Experience: Previous Primary Care and/or Adult – Geriatric experience preferred.
