Job Description

Collin County Adult Clinic is seeking a new Physician Assistant/Nurse Practitioner to provide comprehensive primary care services to low -income patients of all ages in a supportive, collaborative practice. Must be a self-starter and have experience and passion for underserved patient populations.

Required Experience:
• Licensed as a physician assistant/nurse practitioner in the State of Texas.
• Minimum 3 years clinical experience as a primary care nurse practitioner/physician assistant
• Spanish proficient
• Enthusiasm for and commitment to working with vulnerable population
• Working knowledge of Federally Qualified Health Centers and the Texas Medicaid 1115 Waiver
• Ability to provide efficient yet compassionate care
• Proficiency with electronic health record charting
• Strong verbal, written, and interpersonal communication skills are also required.
• Ability to coordinate care team activities and operationalize a medical home for patients with chronic conditions
Compensation: Will be determined by skills and experience
