Founded as a small family practice in Escondido 1932 by Dr. Martin B. Graybill, today we’re the region’s largest Independent Multi-specialty Medical Group. Our capacity to provide for our patients through the full range of care continues as we look to fill the following position:

Physician Assistant or Nurse Practitioner to provide oversight of patient care in the contracted Vista and Oceanside Skilled Nursing facilities working under the direct supervision of the SNF medical director MD. This would include rounding, taking calls, recommending course of action for patient’s health concerns, communication with primary care provider and appropriate documentation. The position is Monday through Friday.

Skills/ Requirements
Skilled Nursing Facility based patient care experience is highly preferred. The incumbent must hold a current California (PA or NP) license. At least 2 years prior geriatric care experience required.
